Confluence Cloud site rename support

When performing a Confluence site rename as described in the Atlassian documentation, the content which was embedded using the macros of SharePoint Connector for Confluence will be correctly rendered on the newly renamed site.

The app needs to be reconfigured on the renamed Confluence site.
